Aim

The aim of this assessment is for you to demonstrate an understanding of effective nursing care planning. Nursing care planning enables nurses and student nurses to demonstrate their clinical decision-making capabilities and supports safe evidence-based practice.

Learning Outcome 1: Demonstrate theoretical knowledge, practical skills and cultural capabilities that underpin safe and competent nursing care for people living with health alterations.

Task Description

For this task you need to create TWO nursing care plans (NCP) for Mrs. Beryl Atkinson (see case study below). You explored how to develop nursing care plans in Module 1, Topic 1.2: Planning Effective Nursing Care. Refer to this module to help you prepare the plans for Mrs. Atkinson.

Instructions

Examine the following case study, and with reference to appropriate scholarly literature:

1. Identify TWO (2) actual or potential nursing problems relevant to Mrs. Beryl Atkinson. For each problem, justify your choice of the problem (e.g., by referring to the patient’s assessment data in the case study). The two (2) problems form the basis for the two (2) care plans, i.e., one problem for each care plan.

2. For EACH nursing problem, identify ONE (1) SMART goal for care (total 2 goals). Remember a SMART goal is specific, measurable, attainable, relevant and timely. 

3. For EACH goal, describe TWO (2) nursing interventions you would implement to meet the goal (total 4 interventions). For each nursing intervention, provide a rationale by referring to appropriate literature. 

4. For each intervention, describe the expected outcome and how you will evaluate the effectiveness of the intervention (i.e., how you will know that the intervention has/has not worked, by referring to appropriate scholarly literature.

Case Study

Mrs. Beryl Atkinson is a 72-year-old woman who moved to Edgewater Residential Aged Care Facility in Brisbane, Queensland, 3 months ago. Mrs. Atkinson is a Butchulla woman and Aboriginal Elder, she prefers to be called Aunty Beryl or Aunty B. She is a widow with two adult children, a daughter, and a son, who both live in Brisbane. Prior to coming to the residential aged care facility, Aunty Beryl lived independently in her home, which was in the regional town of Hervey Bay where she grew up. Although the move to Brisbane has allowed her to be closer to her children (who both live nearby to the residential aged care facility, and visit regularly), she isn’t able to see her sisters and her large extended family each day, which she used to do. Because of the move, Aunty Beryl had to give up her volunteer role at the local First Nations’ community centre, where she taught traditional basket weaving and Butchulla language.

Since arriving at the residential care facility, Aunty Beryl’s appetite has been poor. She refuses meals regularly and says that she does not like the food at the facility. She must be encouraged by staff to drink water regularly. The staff at the facility are worried about her recent weight loss (5kgs in 3 months).

Aunty Beryl is unsteady on her feet and requires ‘one on standby assist’. She uses a walking frame when mobilising. Since arriving at the facility, Aunty Beryl has been wanting to spend increasingly more time in bed or sitting in the chair in her room. She complains of pain in her left hip and is reluctant to mobilise.
