Question 1.
Working in groups during the semester, a risk management plan was developed by you for a particular hazard for a specified context.
a) List in ‘dot points’: (i) for whom the report was written, (ii) who you were pretending to be as the author of the report, (iii) the hazard addressed, and (iv) the context for the plan.
b) You analysed and rated the level of risk (e.g. severity versus likelihood) and then made recommendations for how to address the hazard using hierarchy of controls. Describe the recommendations and explain how you deemed them in relation to reasonably practicable standard.
c) Explain how your recommendations for the hierarchy of controls met the requirements of the PCBU’s primary duty of care clause.

Question 2.

One way of explaining safety culture is by referring to values, attitudes, assumptions and beliefs that employees and management hold towards issues os safety. Referring to any of the case studies (BP Texas City oil refinery explosion, Piper Alpha, Waterfall, Rana Plaza and BP Deepwater Horizon), discuss how theirvalues, attitudes, assumptions and beliefs toward safety culture, was inferior to that of operations in highly reliable organisations (HROs).

a) Work Health and Safety Act 2011 (NSW) stipulates that workers must be consulted regarding decisions about safe working practices. Discuss when, why and how organisations must involve workers in WHS safety decision making
b) There is international consensus that worker involvement and participation (WIP) greatly assists in creating a culture of safety at the workplace. Discuss why WIP should become a legal requirement
