Aim
In this assessment item you will have the opportunity to demonstrate the progression of your critical
thinking skills by identifying a clinical problem and planning the nursing care of a patient from a
selected case study. Nursing care planning enables nurses and student nurses to demonstrate their
clinical decision-making capabilities and supports safe evidence-based practice.
This assessment item will assess:
• Learning Outcome 1: Demonstrate theoretical knowledge, practical skills and cultural
capabilities that underpin safe and competent nursing care for people with acute
medical/surgical conditions;
• Learning Outcome 2: Create an evidence-based plan of nursing care for a person with an
acute medical/surgical condition.
Task Description
Within the online content you have explored acute nursing care and the importance of nursing care
plans. For this task, you need to examine a case study and create an appropriate nursing care plan
for the patient in the case study (1500 words).
Instructions:
Examine ONE of the following case studies, and with reference to appropriate scholarly literature:
1. Identify TWO (2) actual or potential priority problems relevant to the patient described in
the selected case study. For each problem, justify your choice as a priority (e.g., by referring
to the patient’s assessment data in the selected case study and to appropriate literature)
2. For EACH priority problem, identify ONE (1) SMART goal for care of the selected patient
(total 2 goals). Remember a SMART goal is specific, measurable, attainable, realistic and
timely
3. For EACH goal, describe TWO (2) evidence-based nursing interventions you would
implement to meet the goal (total 4 interventions). For each nursing intervention, justify
your choice by referring to appropriate literature.
4. For EACH goal, identify the expected outcomes and describe your evaluation approach
(e.g., how, and when, would you evaluate that your interventions have been effective or
not?).

Case Study:
Identification: Alina Antonova is an 8-year-old child who had her first asthma attack at her primary
school sports day, today. Alina lives with her Mum Angelina, Dad Anton and Aunty Daryna. She is a
very active child and loves to draw.
Situation: Alina was admitted to the ED with her schoolteacher after being transported by the
Queensland Ambulance Service (QAS) from school. QAS paramedics administered 2x salbutamol
puffs via spacer twice on route to the hospital. Her parents have been notified and they are on the
way to the hospital. No one in the family has had asthma before.
Observations: Her vital signs on arrival to the hospital are: HR 125bpm, SpO2 95% on Room Air,
Temp 36.5 ºC, RR 28 bpm. Her GCS is 15/15, face is flushed, and she appears very scared.
Background: Alina was a premature baby born at 35 weeks' gestation. Her childhood vaccinations
are up to date. She has no known allergies. Her family moved to Australia from the Ukraine last year
and they speak English and Ukrainian.
Responsibility: You have been allocated the care of Alina.Other elements:
• You need to adhere strictly to the word limit of 1,500 words. The word limit for an
