Task description:

There are two parts to this assignment. You must complete both parts and submit them as separate documents.

Part A:

For this task, you need to create a digital poster (200 words) that communicates information about a specific health condition that is common in older adults (you can use the condition identified in Assessment 1), and recommend initiatives to optimise the health of older people diagnosed with, or at risk of, complex health conditions and/or cognitive decline associated with the specific health condition.

You MUST include the following content in your poster:

  • Title (highly creative and captures target audience’s attention)
  • Introduction (briefly discuss the topic and the impact on the target population, and provide simple statistics/infographic to appeal to target audience)
  • Determine the factors that increase risk of the health condition in your target population (critically analyse risk in terms of stereotypes, prejudices, and discrimination, changes in the natural, social and governmental environment, and aging processes)
  • Key ‘take-home’ quality improvement initiatives (3-5 essential points related to the prevention and/or control)
  • Where to get more information (list of other resources/websites)
  • In text references [reference list provided separately at the end of your justification and not included in word count]. Written content should be supported by at least 10 high quality scholarly references, and all images/websites must be acknowledged appropriately. You must use the Vancouver numbered referencing style on your poster.

You MUST include the following content in relation to your poster.

  • Justify your selection of the audience, included content, implications for nursing care related to change in environment, and the relevancy of selected poster features for your target audience
  • Evaluate the implications of the quality improvement initiatives on agism (stereotypes, prejudices, and discrimination), changes in the environment (natural, social, and governmental), and aging processes that have an impact on the health of older people.

In this task, you will provide clear evidence of critical thinking and consideration of the applicability and relevance of the poster to the target population.
