Project Goal:

The goal of the educational package is to bridge the gap in knowledge and to provide evidence based support and information to the new and junior clinicians within the ICU, which will help them to implement a best practice approach in assessment, management and prevention of delirium in patients who are admitted to ICU.

Project Problem:

Delirium is an acute onset of fluctuating confusional state which is a very common psychiatric disorder in critically ill patients in ICU. Prolonged admission to ICU, infection, extensive usage of opioids, hypoxia, dementia and severe medical illness are the common precipitating factors for developing delirium (Amado & Kallenbacha 2017). Prevention, early intervention and appropriate management of delirium have positive impact on healthcare system which includes clinical and cost benefits (Annelies Wassenaar et al 2018). The other benefits are reduced days of hospital stay in acute care settings, decreased pressure on patient and patient’s family members and will increase patient satisfaction (Jing Wang et al 2017). In addition to these, delirium predisposes increases the risk of deaths of patient during hospitalisation, can cause long term cognitive impairment and physical disability (Lee et al 20118). Though there are various delirium assessment and screening tools available, more often there is a failure in detecting the development of delirium in ICU. I have observed that time restriction and lack of knowledge about the importance of early detection and management of delirium, accessing the appropriate resources and busy clinical environments in ICU are the common barriers. The lack of awareness in new junior medical and nursing are professionals is another issue which was identified within ICU in managing patients with delirium (Bannon et al 2018). Though there are multiple management guidelines and policies available within the department resistance to change from the holistic approach in relation to the management of delirium is also an obstacle. To eliminate the gap in knowledge and to provide the best patient centred care, induction of revised and evidence based delirium management educational package which is purely focused on patients who are critically ill is essential.
