John is a 69-year-old married man with 2 children. John suffers with COPD, diabetes, chronic heart failure, poor circulation and who also has an aneurism in his left groin area. John is very unsteady gait at times; he can only walk short distances with the aid of a walking frame, so he needs his wheelchair on most occasions. The reason for his unstableness is due to having two toes on his left foot and one on his right foot were amputated due to his poor circulation combined with the poor circulation. John cannot be left alone for long periods of time due to his conditions. He has several medications to take daily and as he is quite forgetful his wife Jane gets his medication ready for him each day and reminds him when to take them and at what times. She also reminds him to check his blood sugars to ensure he is taking the correct does of insulin. John had to retire early due to several health conditions. He was a motor mechanic and enjoyed his work and had planned to work right up until retirement. His wife Jane provides is his main care and finds this difficult at times and has assistance from her son who lives nearby. Socially John used to enjoy playing cards at the locally community centre with friends. In addition, he used a men’s shed weekly which he really enjoyed. He has not been able to attend more recently due to mobility difficulties and transport to the venue.
