Changes in self-management skills and behavior may subsequently lead to an improvement in a number of factors including quality of life, body mass index, blood pressure, and glycaemic control. The purpose of this study is to evaluate the effectiveness of the intervention program using a self-management skill development program to improve their health status. This will provide a better understanding of the risk factors associated with diabetes in adults, and develop suitable prevention programs to promote health in patients with type 2 diabetes
Objective: To examine the effectiveness of self-management skills-based intervention program on improvement of knowledge of diabetic conditions, obesity, blood pressure, quality of life and reduction of glucose and HbA1c among people with type 2 diabetes.

Understanding importance of self management skill development and its related risk factors associated with diabetic condition in older adults is critical for prevention programs development to further improve health status and prevent high incidence of chronic diseases.
Subjects Data were collected from over 409 patients from China. Eligible subjects are people aged 45 and above and reported one of chronic disease history or patients who were under medication.
