Antonio

Antonio is a 69-year-old man who has had Type 2 diabetes for four years. He is a retired librarian who lives with his wife and dog. He has two adult children who are living in different states. He has been overweight most of his life and is currently classified as obese with a BMI of 38. His father was a Type 2 diabetic and died from a stroke at the age of 70. 

To manage his diabetes, Antonio attends his local Diabetes Education Centre where he receives advice and guidance from the multidisciplinary team to help make informed choices on his lifestyle. His wife attends these education sessions when she can to help support her husband.

He has been prescribed the following medications: 

  • 500mg Metformin BD 

  • 120mg SR Gliclazide mane 

  • 50mg Metoprolol BD 

Antonio has been encouraged to take his BGLs twice daily, but he often forgets. He attends his local GP every three months to have his HbA1c levels checked. Every 12-months he has his General Practitioner Diabetes Management Plan Reviewed. 

Antonio had planned to visit the local Diabetes Education Centre today, however his at 0900 hours his wife called them to let them know that he had developed some abdominal discomfort, nausea, vomiting and diarrhoea and would not be attending his session. Antonio remained in bed for the rest of the morning. He could not take anything orally and was complaining to his wife of feeling lightheaded and unwell. At 1700 hours Antonio remained unwell with constant vomiting and diarrhoea. He was unable to communicate more than a few words and was drowsy. His wife immediately called an ambulance.

1730 hours: Antonio has arrived in the Emergency Department. He is confused, drowsy and has slurred speech. He wakes with stimulus, is pale, cold and clammy. His wife is present, and she is very distressed.
