Case Study:
Greg, a 55 year old man was brought by his wife to Emergency Department. He came with complaints of sudden onset of central chest pain one hour ago. He complained of breathlessness, chest tightness and diaphoresis.
Central chest pain is squeezing in character, 8/10 score (Glasgow pain scale) in severity, lasting an hour, radiating to the left jaw and to the left arm and to the tip of the little finger. It is associated with diaphoresis. Greg tells you that the pain feels worse when he moves. It is not relieved by any means. It is progressive in nature.
Greg is a chronic heavy smoker for 20 years. He has had hypertension for 10 years and diabetes mellitus for 3 years. He is a social drinker but not more than two standard drinks per day. He is working as a bank manager. No history of hospitalization. Family history of hypertension and diabetes mellitus in maternal side. No known drug allergy. He is taking regularly
ACEI (Enalapril 5 mg p.o),
Metformin 500 mg twice daily,
multivitamin (OTC) once a day.
Greg is anxious, pale, sweating and in pain.
On examination:
• Oxygen 92%,
• Temperature 38?C,
• HR 94/min,
• BP 160/90 mmHg,
• I + II (+), no murmur.
• Respiratory rate 28/min,
• VBS (+),
• basal crepitations (+)
