The request is for the community nurses to provide home visits for the daily dressing to Judy’s leg ulcer, and the request was from the ALO, as AMS are not able to attend to this need. The ALO is unable to come to the first home visit for introduction. The accompanying discharge letter from the hospital states that she may be eligible to be registered for CTG.
