You have been called to the home of 54-year-old Matthew Smith. On arrival you find Matthew lying in bed on his side with a cool wet cloth over his head. His knees are pulled up towards his chest. His daughter Sara is in attendance.
Matthew is slightly confused so his daughter informs you that her father has been complaining of persistent generalised abdominal pain over the last 24 hours increasing in severity. She states he has been vomiting intermittently throughout the night and has not eaten. He is now unable to tolerate oral fluids.
Upon questioning Matthew is unable to localise the pain, however he states it had been severe in his right lower quadrant and was “on and off” for several days.
On examination Matthew’s entire abdomen is rigid and tender on palpation. He appears flushed and diaphoretic. He states his pain is severe. He last had his bowels open yesterday and has no pain or burning on urination.

GCS 13 (3/4/6)
Temperature 40.3oC
Skin Warm to touch, clammy
ECG Sinus tachycardia
Pain 9/10 and constant
Abdomen Rigid and tender
