On assessment it is observed by the nurse that A.W. has a golf ball sized abscess to her left ankle that is malodorous with moderate amounts of serosanguinous drainage, bruising and track marks to her bilateral antecubitals, and cellulitis to her right wrist. A.W. is alert with voice stimulation and oriented to self and situation.
