Scenario — 'Dean', is a teenager transferring to Hospital surgery from acute care within GP Practice. 

Dean is 16 years old and presents at his Family Doctors clinic with a deep laceration in his r ght forearm and severed tendons to his hand after punching through a plate glass window. He sustained this injury during a drunken night out whilst celebrating his brother's stag do (a party to celebrate his brother becoming engaged, prior to getting married). Dean is 102 kilograms. 1.88 metres tall, dressed in casual jeans and an expensive jacket (that he doesn't want to remove or loose), and presents as aggressive and a bit intimidating because of his language, size and rough appearance. He is schedu:ed for emergency surgery at the local Hospital later today, but he has been described by other staff as a 'problem patient' since arrival, swearing and shouting and demanding increasingly large doses of painkillers and reporting no effect from the analgesia (pain medicine) he has been given. He has made himself extremely unpopular with the staff and none of them go into his single room unless they really have to. None of his friends had come in with him, but are out in the car park waiting. His mother has been rung and is on her way but has not yet arrived. He will be transferred to an ambulance and then taken to the hospital soon for surgical repair of his arm under general anaesthetic. The Doctor has asked you (the HCA) to "keep him quiet, sort him out and get him ready to go to hospital". But you are unsure if that is a realistic part of your role? What can you do here to be safe (for yourself and for Dean) but also practically help Dean and the clinical team give effective care? 
Once you have discussed your concerns about what the Doctor has told you to do with the Registered Nurse, the Nurse states that they will lead and direct the care for Dean and the preparation for the transfer to Hospital by the Ambulance team. However, the Nurse wants you very involved in the delegated care for Dean and also wants you to participate in the verbal handover to the Ambulance staff using the communication tool called 'SBARR' to do this.

2. Assessment & Planning -using 'goal centred' care for Dean -Describe how you provided practical support to this yourg person physically and emotionally (what ore the needs you are wanting fa practically help him with; and how wilt you monitor and measure his condition for signs of improvement or worsening). 
• To achieve this, you will list and describe 4 priorities of care (x1 for his physical needs and xi fo• his emotional needs) that you can address as an HCA to show how you can have a positive impact in the practical support of th s young person, (while staying within your scope of professional practice). This is where you can set goals to address these needs effectively; show what and how you as an HCA support Dean's needs. Consider including details of the tools used and how you measure / monitor the changes? 
3. Analysis - of the difficult interpersonal & care dynamics . Now reflect on the following aspects of this challenging situation - Describe any potential barriers to achieving the support care goals for this young man, such as: - 
• Describe what being 'Professional' means in how the HCA can address challenging care situat ons or people labelled by others as "different or difficult'? 4.
