Students will use critical thinking to assess a community, and will analyze data to identify a priority community health concern. Students will then apply current literature and community reports to explore the implications, interventions, and community supports related to the identified community health concern.
 

Assignment Guidelines:

1. Identify a community of choice that you will work with (ex. Whitby, Oshawa, Ajax) Describe the boundaries (street names) and provide a map (Appendix A) with boundaries outlined. You may choose a subset of a community (ie. South Oshawa), as long as you indicate this on your map.

2. Do an assessment of the community by completing the windshield survey tool in your textbook. Provide at least two data points for each area of the windshield survey (pages 259-260). This will require going out and exploring your chosencommunity safely by foot, transit, or car. (Windshield Survey is in chart format, as Appendix B, see sample below)

3. Identify ONE “community health concern priority” for your community (food insecurity, homelessness, opioid crisis, teenage pregnancy, pandemic, etc.)

4. Next, research the topic of interest, utilizing three recent community reports or databases within the last 5 years, and three recent scholarly articles from the literature on the topic.

5. Write a scholarly paper describing the issue (background), the implications on the health of the community, the specific supports provided in the community, and the role of the community health nurse in supporting this issue in the community. See textbook chapter 9. Describe at least ONE CHN Standard and how this is demonstrated in regards to the topic of interest.

6. Provide a LIST of support services in the chosen community, relevant to the topic of interest. This list must include at least 6 support services, with name of agency, address, hours of operation, and a 2-3 sentence description of the service. Include this list as Appendix C.
