Anthony Smith is a 58 year old man who lives in an outer suburb of Brisbane. He is widowed and lives alone, renting a small house. Since his wife died 12 months ago Anthony has found it difficult to move on with his life. He suffers with a chronic pain condition due to a back injury sustained several years ago when he fell off the roof of a house he was working on in his lifelong career as a roof tiler. This resulted in him requiring emergency surgery that resulted in three lumbar spinal fusions which ultimately forced him into early retirement and receipt of a disability support pension. 
Since his injury 5 years ago Anthony has gradually increased in weight and his BMI is currently 29. He has also been diagnosed with hypertension and hypercholesterolaemia. He does not smoke, but has the occasional social drink if he does visit his local bowls club. However, since his accident he has not been able to play bowls or participate in any form of regular exercise. He has one son who is married and lives in a regional town in Central Queensland. Anthony feels quite lonely and wishes he could see his son and daughter-in-law more often. When asked about his daily living patterns, he admits that he often can’t be bothered cooking ‘’proper’’ meals relying instead on snacks and take-away food instead. You are working as a Community Nurse in the Health Centre where Anthony has been referred.
Develop your essay response to this scenario using the following guidelines:
Write an introduction that introduces ‘Anthony’ and outlines the issues that will be addressed in your case study (e.g. 150 words)
Outline the impact of cardiovascular (CVD) disease in Australian males (e.g. 250 words). Your outline should include current prevalence and mortality rates, impact on health, gender differences, evidenced with literature.
Describe social and behavioural determinants that increase the risk of developing CVD and how they relate to this case study – evidenced with literature.
Three of the five pillars of health promotion (as described in the Ottawa Charter (WHO, 1986) are ‘supportive environments’, ‘reorientation of health services’, and ‘community action’. Choose one of these three pillars and describe a potential health promotion action based on that pillar that you and Anthony might work on to address his situation supporting your ideas with evidence from the literature.
Drawing on the fundamental processes and core skills of Motivational Interviewing, give examples of how you would engage Anthony, as a recipient of Disability Services, in a discussion about the potential health promotion opportunity identified above.
Write a conclusion that summarises your key points.
