The incidence is increasing and in the United States alone it is recognised as the leading cause of admission for gastroenterology/hepatobiliary issues (Lankische, Apte & Banks, 2015, p. 85). The management of pancreatitis is complex but nutritional support is vital as it aims to prevent the absorption of endotoxins. Traditionally total parenteral nutrition (TPN) aimed to rest the bowel and avoid pancreatic stimulation though it is now recognised that this may lead to gut atrophy and translocation of gut bacteria. Enteral nutrition (EN) is an alternate form that canpreserve gut mucosa but may be uncomfortable and carry its own side effects like diarrhoea and vomiting, exacerbating the problem. The following case study examines the management of a patient with pancreatitis with TPN.
 

Patient Background

Mr X, a 50-year-old male presented with a five day history of abdominal pain. The pain was associated with nausea and vomiting but Mr X denied any fevers, diarrhoea, or weight loss. Mr X had a recent admission for uncomplicated acute pancreatitis three months prior and one previous admission for necrotising pancreatitis required drainage of a collection. Concurrent medical conditions include gastro-oesophageal reflux disease, chronic pancreatitis and depression and past medical history included a hernia repair and previous chronic consumption of alcohol (4-6 standard drinks a day) which ceased a year ago. Documentation showed allergies to statins and Mr X stated pantoprazole as an allergy that  was subsequently recorded.
