Scenario 
Arika Bunda is a 35-year-old Indigenous woman who resides with her partner and 2 children. Arika is a Wiradjuri woman and has a very close connection with her country, immediate and extended family. 
Three years ago Arika had significant periods of unwellness which were often gynecological in nature, speaking with Arika, she stated that she "had always has those kind of issues, even as a teenager". However due to the level of persistence in relation to her periods of unwellness Arika decided to have her symptoms further investigated. Arika was diagnosed with uterine cancer at the age of 32. 
Under the guidance and advice of specialists Arika and her family decided that a Hysterectomy was the appropriate course of action with a follow up of Chemotherapy commencing 6 weeks post-surgery. In a scan performed prior to the commencement of Chemotherapy, it was discovered that the cancer had metastasized to her bowel however the chemotherapy would go ahead, the introduction of radiotherapy was also discussed and decided. 
Arika and her family commenced the process which would be 8 months in duration with regular scans throughout to assess the success of the treatment in the reduction of the secondary tumor in Arika's bowel. Initially, within early scans it appeared the treatment was being somewhat successful, however at the 4 month mark significant cancer markers were evident within results and the cancer had further spread to Arika's spine, Arika and her family were given a terminal diagnosis. 
Student Context 
You are a first year nursing student and have taken over the care of Arika. (0700 on the 21/10/2019) Arika has been brought into the hospital in the early hours of the morning by ambulance. You receive the following hand over from the nurse transferring her to your ward. 
"Hi I'm Jennie Hills and I am the RN who has been looking after Arika in short stay. She came to us from ED whilst waiting for x-ray and blood results. She has been reviewed by the RMO and charted IV AB's for CAP and is required to have the first dose ASAP. Arika has a history of Uterine cancer with metastases in the bowel and spine. I have offered fluids but she has declined to drink anything except for to take the nurse initiated paracetamol at 0600. The last vitals taken are giving her a MEWS of 4 which the RMO is aware of and has advised to give the AB's as soon as possible and he will come to the ward to review on the morning round. Arika has been having oxygen therapy at 4 litres per minutes via nasal prongs and is alert and orientated."
