History of present complaint: Babes had a two-day history of fever. sweating a. productive cough. Babes had influenza a week ago. Her past medical history is type 2 diabetes for which she takes Glibenclamide once a day. This morning she became increasingly breathless and slightly confused. 

A provisional diagnosis: a possible community acquired pneumonia is given. 

On assessment: You note she has a productive cough. (of half a cup of yellow sputum). and is unable to speak in full sentences. Babes also has reduced air entry to the base of her left lung.
