Background

Beatrice Spencer is an 86yr old woman who has dementia. She lives with her 84yr old husband who is her main carer. She occasionally mobilises around the home using a four-wheeled frame, but often forgets to use it. Until recently Mr. Spencer has been caring for her independently, with the couple's children delivering groceries to the home twice a week. However, when Mrs. Spencer was recently admitted to hospital for a few nights with pneumonia, the couple's children reported to nursing staff that they were concerned that their father was beginning to struggle at home. They stated they Mrs. Spencer has recently become less predictable as her dementia worsens. When the nurse addressed this with Mr. Spencer he stated he feels like he's unable to provide the support that she requires. The nurse has asked you to become involved as the care coordinator prior to Mrs. Spencer's discharge. When asked, Mrs. Spencer is unable to tell you about her home situation or current medical history. Mr. Spencer tells you his main concerns relate to ensuring Mrs. Spencer is safe when he's doing housework. He also struggles with assisting her in/out of the tub for showers and ensuring she takes the right medications at the right time, particularly because she struggles to swallow pills and frequently refuses them.

Current medications • Frusemide 20mg twice daily • Panadol Osteo 665mg three times daily • Exelon (Rivastigmine) 10 transdermal patch daily • Perindopril 2mg twice a day • Simvastatin 40mg daily • Esomeprazole 40mg daily • Warfarin 2mg daily (with dinner)
