John is a 70-year-old retired accountant. He presents to the hospital emergency department with the sudden onset of chest pain, breathlessness, tingling and numbness in his left hand and heaviness along the inner border of his left arm. On questioning by the doctor, he mentions having pain in his legs when he goes for a walk, and is finding that his breathlessness occurs from the slightest exertion. He said that the discomfort in his chest is aggravated if he does any physical work. 

John has been living alone for three years since his wife passed away. 

He has three children. One daughter lives close to John and is very supportive. 

Medical History: 

Type 2 Diabetes, diagnosed 10 years ago. 

Hypertension, diagnosed 5 years ago.
