In Part 1 of this class you would have used manual clotting techniques to provide a ‘clotting screen’ for your patient which involves measurement of the Pro-thrombin time (PT/INR) and the Activated Partial Thromboplastin time (APTT). You would have performed these tests on a quality control (QC) plasma sample to ensure the accuracy of your technique and once you had obtained times that fit within the reference range for the control, you would proceed to test your patient sample.

Part 1 Assessment tasks 

• Explain the underlying principles of the tests used in determining the INR and the APTT ratio.
• Tabulate the clotting times for the control sample and patient 1
• Discuss the possible pathological causes responsible for the patient’s results

In Part 2 of the practical class you would have been asked to carry out a factor IX assay on another patient (Patient 2) which uses the same manual technique but on several dilutions of the patient’s plasma. Factors assays work on the principle that:
1. The APTT has a clear mathematical relationship with the concentration of coagulation factors.
2. If the only variable altering the APTT of a sample relative to a ‘normal’ reference plasma sample is the level of a specific coagulation factor then the value of the APTT can be used to calculate the concentration of that factor.
