Case scenario 2: Breastfeeding di?culties for the obese woman

Ava is a G1P1, with a negative blood group and BMI of 38 who has attended all scheduled antenatal visits in a shared care model with a midwife and obstetrician. Ava had a caesarean section following prolonged labour at term. It is noted from her history that she had an epidural and oxytocin infusion commenced when she was 5cm dilated. Several hours later and with no further progress, the CTG indicated fetal distress, and Ava was also exhausted. Ava had a PPH with blood loss noted to be 1000mls, her bleeding has since stopped and her lochia noted as normal.

You are caring for Ava on the morning of day 3 postnatally and she is visibly upset. Ava asks if there is anything she is doing wrong as her baby Charlotte is very unsettled and her nipples are sore. She also asks you whether she should expect "anything else to go wrong". Please discuss this case in terms of breastfeeding di?culties and the obese woman, acknowledging any other factors that may be impacting her lactation.
