Bolt and Catch Pty Ltd is a large manufacturer of gate bolts and hinges for commercial and agricultural use. The company has been in business for seventy years and employs a staff of 120 people ranging from managerial, marketing and sales, engineering, trade and production personnel. You conduct the workplace inspection of the factory. On entering the ‘Press Room’ you notice signage indicating “Safety Equipment must be worn in this area”. Some of the operators are wearing face masks, hearing protection, safety boots and gloves, some employees are wearing an assortment of protective equipment, some none at all. You find the three new Chinese presses, which are operated by the same number of employees – none of whom are wearing any protective equipment. It is extremely noisy in the ‘Press Room’ and the three operators have to continually shout at each other to be heard above the noise emanating from all of the presses.

A major insurance company has merged with another company’s Call Centre employing 120 personnel on a rotating three-shift basis. The Call Centre is located in the CBD of a major city. The Call Centre has been run down in terms of equipment and support facilities before
the merger.
