Presenting complaint
Site: Central to left low back pain.
Quality: Constant ache with sharp, pinching pains on movement.
Intensity: 12/10 (!) at worst and at least 6/10 constantly.
Radiations: Aching pain extends from the central lumbosacral region into the left
buttock.  Sharp pinching pain on movement is local to the lower spine.
Timing: Onset was two and half weeks (17 days) ago.  Hakim spent the day
 

helping a friend to lay floor boards.  He was a little stiff and sore in the low back during the day but that is not unusual when he does heavy work.  By the time he went to bed that night, the pain had increased in intensity, he had sharp back pain when moving and was having difficulty getting up and down from the couch.  He first noticed pins and needles when lying in bed the next morning after a very disturbed sleep. Since onset, Hakim has not worked and has been doing very little activity as he is afraid that it will make his back pain worse.  
 

Neurological: Pins and needles in the left posterior thigh to the knee.  Pins and needles come and go with no apparent pattern.
 

Aggravating factors: Sitting, moving, any activity.
 

Relieving factors: Nothing.  
 

Investigations and Treatment: Hakim saw his GP the day after his injury.  The GP ordered a plain film lumbar spine Xray, which was unremarkable, and advised that Hakim had sprained his back he should rest until it heals.
 

History of Presenting Complaint: Hakim has a long history of mild aches and pains in the low back associated with his work.  He has not experienced episodes similar to his current complaint.
 

FURTHER INFORMATION
Hakim has not worked since the onset of low back pain.  He has been employed by the same company for over 20 years and has hundreds of hours of sick leave to use.  
Hakim has spent most of the past 2 weeks lying either on the couch or in bed, watching Netflix.  He is able to perform basic self care tasks, such as toileting, dressing and showering but his wife has been doing all of the house hold and parenting tasks and bringing his meals to him.   Hakim explains that he has to rest as much as possible so that his back will heal.  He has a friend who had back surgery last year and he is afraid that his situation might be the same and he may never recover.
 

Hakim is sleeping poorly at night and napping often during the day.
