Warwick is a 56-year-old indigenous man, married with three teenage children. He is one of the elders in a large community in the Northern Territory. Warwick is obese, having a BMI of 31.9 kg/m2. He does not smoke and walks 'around' town twice or three times a week. His favorite food is fish and chips from the local take-away shop. He usually washes this down with a litre of lemon squash. Some blood tests were ordered upon a GP visit to check his cholesterol and fasting blood sugar and to do a glucose tolerance test (GTT). The results showed that he had a serum cholesterol level of 7.2 mmol/L and a fasting triglyceride of more than 4.7 mmol/L. His fasting blood sugar was 7.7 mmol.L.

On further physical examination of Warwick, the doctor finds that he has the physical marker for acanthosis nigricans. The GP informs Warwick that he now has type 2 diabetes. It is absolutely vital for him to eat more fruits and vegetables and less fat and salt, exercise more and lose weight. The doctor also prescribes glibenclamide 5 mg to be taken each morning after breakfast. In addition, he asks Warwick to see a diabetes nurse educator (DNE) within the next week for further education.

During the follow-up visit, the DNE educates Warwick on the management of this diabetes. She shows him a video and speaks to him about the importance of his diet. She encourages him to include more of the traditional indigenous foods. This means that he will eat much less processed food, which is loaded with fat, sugar and salt. She gives him pictures of familiar indigenous foods that he might include in his diet. The DNE explains to him the importance of testing his for glucose levels for the first month so that she can monitor his blood glucose level (BGL) trends. She suggests that he do this once a day as soon as he gets out of bed. She gives him a portable blood glucose monitor and demonstrates how to use a disposable small lancet to obtain blood from his finger and apply it to a test strip. She tells him the normal fasting blood glucose level and explains what levels he should report to the doctor immediately. She explains to him and gives written instructions about how to recognise hypoglycaemia and hyperglycaemia, and what to do should he experience either of these. The DNE’s instructions include the need to take a further blood sugar test if he felt unwell. She also asked him to keep a daily record of these tests and bring it with him to his next appointment in two weeks.

Warwick has maintained his lifestyle modifications and over the following year has lost 25 kg. He follows the instructions and regularly follows up with the GP and DNE. As he has reduced his BMI through this healthy diet, his diabetes remains stable. This has been confirmed by regularly testing of HbA 1C. Warwick has sustained this change with a lot of help and support from his wife and children, who have also adopted these lifestyle modifications. They are all feeling much healthier now.
