This is Star’s first pregnancy. She is 5’3” and 168lbs at this visit. She has gained 18lbs since the start of her pregnancy. Her vital signs are: B/P 130/88, P 70, R 18, O2 99%, Temp 36.8C. You also know that Star’s father developed type 2 diabetes 2 years ago. As you share the test results with Star, you notice that she is quiet and not making eye contact. After a quiet moment Star looks at you with tears in her eyes and asks if the diabetes is her fault and if it will hurt her baby.

You are the RN in a 4 room Primary Health Clinic in Surrey. Other than the medical office assistant (MOA) you are working alone. Your shift begins at 1230 and you walk into the clinic just after 12 noon knowing that you have a schedule that is completely full of well baby visits for immunizations and baby weights. As you walk past the desk you notice a teenager dressed with untied shoes, dirty old pants, and a tight t-shirt. Her hair is uncombed and you can see a tattoo on her neck. She is carrying a very large plastic bag that appears to be full of clothing. You hear the MOA tell the teenager that there is no space for her. As you slow down, the MOA also tells the teenager that no there is no bed and certainly no food at the baby clinic. You watch as the teenager turns to leave. As you change direction to talk to the teen the MOA sharply reminds you the clinic is full and people are waiting.
