Case Study 1 – Angela

Angela is a 55-year-old Caucasian woman interested in a surgical procedure to treat jowling and a double chin. She feels she has lost the definition of her jaw line and is developing marionette lines. On observation, her skin has lost elasticity, though this seems only apparent on the lower areas of her face.
The sub-mental area presents a small pocket of fat though no excess skin. The recommended surgical procedures were discussed and the patient received supporting information. Angela is booked for a consultation with the doctor in one week’s time to clarify the suitability of the discussed procedures.
Case Study 2 – Geoff

Geoff is interested in finding out some information about rhinoplasty. He has been considering the procedure for a number of years as he views his face as being out of proportion. With observing Geoff’s facial angles, it appears the dorsum has a visible hump and the naso-labial angle is low making the nose appear hooked. Also, the chin is slightly receded causing the nose to appear larger than it really is. We discussed the operation(s) and viewed some pre-post photos. Geoff has received information and is booked to see the doctor for a consultation.

Karina is a 30-year-old Caucasian woman wanting a surgical procedure for pockets of fat on her thighs and cellulite on her thighs and buttocks. Although she wants a surgical procedure, she is concerned with the scars resulting from the surgery as her Caesarean scar has become hypertrophic. The surgical options have been discussed and Karina has received some reading material. She is booked to see the doctor.
Case Study 4 – Leah
Consultation with Dermal Therapist
Leah is in her mid-fifties and is concerned with what she refers to as “tired looking eyes” and would like to discuss surgical options. On observation, Leah’s right brow is lower than her left and this seems to be causing a hooded appearance of the right eye. Also, the lower lids are moderately wrinkled due to excess skin. Leah also has crows feet she would like treated though has heard that botulinum toxin is poisonous, though is unsure if this is factual. We discussed her options and she received supporting information. Leah is booked for her consultation with the surgeon.
Case Study 5 – Courtney
Consultation with Dermal Therapist / Cosmetic Nurse
Courtney is 42 years old and wanting to know more about breast surgery. Her main concern is loss of fullness and she mentioned that she has always had small breasts (B cup) but after her child she is now an A-cup. She would like to feel more feminine and be able to wear a wider variety of clothes. Courtney would also like to improve the appearance of her décolletage prior to surgery. Courtney has received further reading material and is booked to see the doctor.

Case Study - Jodie
Jodie is a 52-year-old Asian lady interested in surgical facial rejuvenation. Her main concern is platysma banding of the neck, although she would also benefit from an S-Lift to re-define the jaw line, she feels she is not ready for two surgical operations at the moment. The recommended surgical procedure was discussed, written information was provided and Jodie is booked to see Dr Brown.
Suitable surgical options for Jodie
This visible neck banding indicates a separation of the platysma muscle and a loss of tone resulting in the appearance of two bands running down the front of the neck (Tardy & Thomas, 1995). Jodie would be most suited to a platysma plication operation.
Expected post-operative results
The expected post-operative results with a platysma plication operation will reposition the platysma muscle to its original position. This will significantly reduce or eliminate the appearance of the neck banding and the excess skin under the chin resulting in a more youthful cervicomental angle (Tardy, et al., 1995).
Surgical approach and technique
The surgical approach for a platysma placation operation involves a small incision (2-3 cms long) under the crease of the chin. Through this incision the surgeon can then stitch the free edges of the platysma muscle together (where they have separated) and stitch the submental section of the muscle to the underlying tissues (Oates, 2018).
The hospital stay and recovery
A platysma plication operation is generally done as a day procedure under twilight sedation, however, some patients may choose to have it done under general anaesthetic. The expected downtime will involve a week off work, though residual bruising and swelling may be present for 2 to 3 weeks.
Additional information
Immediately after the operation bandages are worn for 1 to 2 days progressing to a specially designed facial garment worn continually for a further week (Oates, 2018). This garment will reduce swelling and is recommended to wear at night for as long as the swelling may persist.
Scarring
The healed incision site will appear as a tiny scar, which usually appears lighter than the surrounding skin colour (hypopigmentation). For most patients it will hardly be visible especially as the incision can be done in the natural crease under the chin (Oates, 2018). Scarring appears raised and red initially, then flattens over time.
Pre and Post-Operative Care
Prior to surgery, a pre-operative checklist is performed to ensure the patient is fit and healthy for the operation. This will include taking blood pressure and asking a number of questions in relation to the patient current health status.
Complementary Dermal Therapies
A course of 5 microdermabrasion sessions at fortnightly intervals are recommended for Jodie as she is concerned about the fine lines and general skin texture on her neck. The MDA will help to address these concerns and complement the platysma plication procedure.
