Part A
Monitoring and recording support strategies
1. Outline the difference between formal and informal assessment and monitoring strategies you could use to monitor development and determine the effectiveness of a person’s support strategies, and when you would include your supervisor. (50-100 words)
Formal strategies eg notes, behaviour diary, incident report, baseline assessment. Informal startigies include verbal communication between staff and the client, observing. If staff complete notes at the end of each day outlining the activities them and their client particapted in. depending on the goal set a time frame to achieve this goal should be developed. The plan needs to be reviwed periodically.
It's diserable to have the same few staff members work towards the goal with the client so any changes can be observed and notice, EG if a new staff member was to work when the client achieved a new goal, they may not know this is a new action.
If it has been observed for a week that the client is uphappy with their support stratiges I would consult my superviosor.
2. Using the strategies you described in the previous question, you can determine changes in a person’s needs. Once these are identified, you need to report the changes in an individual’s needs and behaviours in consultation with your supervisor.

Example: Avoid jargon Don’t use words that are not understood by everyone. Often workplaces have terms that are only understood by a few people of a group, make sure the language is accessable to anyone who reads the report.
Consideration 1) Notes
Notes should be completed for each client at the end of your shift. They should be factual, consistent and accurate.
Consideration 2)Baseline assessment
Conducting a baseline assessment is a very helpful formal reporting tool. From this you can observe any changes in the clients ability and reflect after 1 week to see if there are any chages. If muliptple staff members are working with a client this is very important so each staff member has a understanding of the clients baseline.
Consideration 3) Behaviour diary
A behaviour diary helps understand the clients baseline behaviour and triggers, I believe this would help assessing the effectiveness of a person’s support stratigies as it can record if they are having more or less behavioirs and draw out any patterns or triggers in their current supports.
Consideration 4) verbal communication
While verbal communication between staff members and the client is very important, it’s important that any information regarding achieving goals either good or bad is reported. This type of Verbal communication is informal.

Refer to the case study of Roman that you worked with in Assessment 2. Read the continued case study below, understand it and reflect on the various issues and challenges it raises both for you as a support worker and for Roman;- the individual you are supporting in a person-centred approach.

You and the support team have been working now with Roman for over 3 months and in that time, his violent verbal behaviour has been observed, recorded, analysed and understood and it is now time for a comprehensive formal review of Roman’s preliminary Individualised Plan with all its component parts – lifestyle enhancement strategies, health and well-being strategies, behaviour support plan, medication regime, nutritional plan, daily activities plan, mobility plan etc.
The analysis of Roman’s behaviour issues have found (not unexpectedly) that his behaviour is connected with- the loss of his partner Peter, the loss of his career as a sportsman and loss of his sense of purpose and individuality. The loss of his family support made it worse together with his fear and frustration at having to adapt to a new way of life that is so different to how it was before. Overall, in your conversations with Roman, he has come to understand that his behaviour is the result of many factors related to unresolved grief, fear and loss in general.
Recently, Roman told you about a program that he saw on ABC TV that intrigued him a lot. It was about an elite female athlete in the USA who lost her legs and the lower half of one arm in a train derailment and who after much therapy decided that she was interested creating a new life by mentoring junior trainees in areas of sporting disciplines related to her previous expertise – track and field.
The emphasis in the TV program was that with vision and commitment, all barriers to life can be overcome or minimised by any person.
When Roman was talking to you about this – he was excited and obviously impressed and stimulated by what he had seen and he made several passing remarks that he would love to be able to do something similar here in Australia. He seemed happy for the first time in a while before slowly slipping back into his old behaviour in the next few hours.
