Chantelle is a twenty-one year old woman who lives in Sunny Valley. She has two children: Daisy (2 years old) and Edwina (1year old) who currently live in foster care. Chantelle is currently going to court to attempt to have the children returned to her.

Health Service. Chantelle was diagnosed with bi-polar disorder two years ago at age 19 after the birth of her first child. Prior to this, she had had depressive symptoms, and had been diagnosed with depression in adolescence. Chantelle has had three hospital admissions in the past year. When unwell, she displays symptoms of mania, is grandiose, irritable, thought disordered and has paranoid delusions.  The psychotic symptoms resolve with treatment, however, in between episodes she has residual symptoms of hypomania. Chantelle responds well to medication, however, is not regularly compliant with medication. She has very little insight into her illness, and does not recognise her symptoms. Chantelle likes feeling ‘high’ and does not want to take medication that will affect her moods. She is currently on a Community Treatment Order and is required to take medication. Chantelle reluctantly accepts the involvement of the mental health service. 

Chantelle lives in NSW Housing accommodation with her partner, Frank who is the father of her two children. They have been in a relationship since Chantelle was 16 years of age. There is a long-standing history of domestic violence in the relationship. In the past, Chantelle and Frank have used cannabis and speed, but both are now attempting to abstain in order to satisfy FACS requirements that they remain drug free. Chantelle has been having urine drug screens to demonstrate her abstinence. However, during this period, they have remained using alcohol, and Frank’s alcohol use is quite high, and can sometimes precipitate the violence he perpetrates in the relationship.

Chantelle is currently unemployed. She has not worked since she was seventeen years of age. She has attempted to undergo some TAFE training, but has not managed to engage in any long-term job training. Since the children were removed, Chantelle has been on an unemployment benefit and has had difficulties remaining on payment as she often forgets to attend appointments. Chantelle has had a Job Capacity Assessment and is not on the Personal Support Program. She was breached for not attending a review in the past.

Chantelle’s NSW Housing home is disorganised, cluttered and untidy. Her washing machine is broken and finds it difficult to get clothes washed. She does not cook and eats mostly take-away meals. Chantelle does not have a routine for household tasks, and the home as become more disorganised since the children were removed. When the children lived with Chantelle, there were concerns about Chantelle’s ability to manage parenting, provide basic needs for the children and establish an appropriate routine.

Chantelle has a close relationship with her sister, Amy. Chantelle has had a difficult relationship with her mother, Barbara, who also has a mental illness. Barbara is quite angry with Chantelle about her ongoing relationship with Frank, and believes that Chantelle has not done all she can to get the children back. Chantelle has lost touch with a lot of her friends, particularly since she has ceased using cannabis. Since her children have been removed, Chantelle feels very lonely and isolated.

Chantelle is now being discharged from the hospital and has been referred for case management to the Sunny Valley Mental Health Team. You have been assigned as Chantelle’s case manager.
