2. Brief
This task consists of one (1) part based on a scenario.
This assessment provides you with the opportunity to demonstrate your knowledge of the following areas:
•Monitoring and reviewing a case management plan
•Closing the case when completed
Part 1
Read the scenario and then answer the questions that follow.
You work at CareShore as a case manager. You are responsible for undertaking case meetings to plan, monitor, and review service provision for clients. In addition, your role requires you to manage work, health, and safety (WHS) implications for the other members of the team, the clients, and yourself in the performance of the service.
Today, you are meeting with an existing client, Maria. Maria is a 64-year-old grandmother who is currently the legal guardian and carer of three of her grandchildren, Marco aged 11, Luca aged 8 and Nico aged 4. Maria became the carer of the children 6 months ago after the children’s mother (Maria’s daughter) became unable to care for the children due to mental health issues. The children’s father is currently serving a life sentence in prison.
When you first met with Maria, she was struggling to care for the children, particularly Luca who had recently started displaying behavioral problems and had frequent violent outbursts. The children’s school had notified the authorities of the issues and had requested Luca be assessed after a spate of difficulties in the classroom. After an initial assessment, Luca was diagnosed with a severe autistic disorder. At the time, Maria said that she was exhausted and although she wished to continue to care for the children, she was not sure that she would be able to.
To address the issues, you:
•Arranged counseling and psychological treatment for Luca to learn to control his outbursts
•Arranged respite care for Maria
•Enrolled Maria in a parenting support program for parents and carers of autistic children
•Located a full-time childcare placement for Nico who was not yet at school
•Made arrangements with the school for Luca to have additional support in the classroom.
1A. As the case manager, you will need to meet with Maria to review her case management plan. What are five things that you will need to establish during the review meeting?
1B. During the review meeting, Maria tells you that the respite care service, which provides one afternoon a week care for the grandchildren, is going really well, but it is on the same day as Luca’s appointment with his counselor (competing and conflicting priorities).
1C: List at least three (3) documents that would be required for the review meeting. Give a description of each document (50-100 words each)
