1. Background/Overview

 

Developing a case management plan is a key aspect of a client-centred approach to case management in community services. To achieve the best possible outcomes for your client, you will need to work with them to establish their goals and identify suitable strategies to meet these goals.
 

2. Brief

This task consists of three (3) parts, all based on a particular scenario.

This assessment provides you with the opportunity to demonstrate your knowledge of the following areas:

  • Case management approaches and legislative requirements

  • Working with the client to set goals and identify rights, obligations and responsibilities based on the client’s needs

  • Develop and assess strategies and services for the client 

  • Document a case management plan

 

3. Questions/Task

You work at CareShore as a Case Manager. You are responsible for facilitating case meetings to plan, monitor and review service provision for clients. In addition, your role requires you to manage work, health and safety (WHS) implications for the other members of the team, the clients and yourself in the performance of the service.

A new client has come to your office requesting help. The client is a young man, Jack, aged 19, who has a history of drug and alcohol abuse. Jack recently found himself in trouble with the local Police after neighbours reported a wild party that he was hosting in his home. The neighbours were particularly concerned as Jack as a two-year-old daughter.

The mother of the child left six months earlier. Jack does not know where she is and has had no contact with her. Jack has been attempting to raise the little girl on his own. Jack’s mother lives nearby and Jack has asked her to help from time to time. However, Jack said that his mother was frustrated with his drinking and drug abuse that often kept her away.

Jack does not currently work and is reliant on a single parent pension to support himself and his daughter. Jack dropped out of high school at 15 and has never had a job. He and his daughter live in a small rented apartment and Jack struggles to pay the rent.

Jack’s greatest concern is that he will lose his daughter. He is keen to make changes in his life so that he can maintain custody of his little girl, but he does not know where to start. He stated that he struggled to maintain the house and didn’t know how to clean or cook properly. He’d always been taken care of by others in his life.

 

Part 1

A. As a case manager, there are a variety of legal obligations you need to meet. Explain how these obligations apply to your management of Jack’s case.

B. To assist Jack, you will need to apply case management approaches. Provide a definition of each of the following approaches and how it could be applied when working with Jack.

C: Provide a definition of each stage of the behaviour change model and provide an example of how Jack might move through each stage.

 

Part 2

2A. As the case manager, you will need to work with a variety of clients with diverse needs. For each group within the client population describe their specific needs and things that need to be considered when providing services.

 

Part 3

3A: Complete the following care plan for Jack (determine the goals and actions based on the information provided in the scenario). When developing the plan, we recommend that you research services within your own geographical area. Your plan must include a minimum of five actions. These can be related to the same or different goals as you see fit. Responsibilities and monitoring strategies must be included for each action identified.
