Case Study

Alfonse is a new person for your service. His health has declined over the past six months to the point where his functional ability is extremely poor. He requires assessment for ongoing support. The result of his ACAT assessment is that he qualifies for a high-level care package.

Physical health: 

• Alfonse has extensive liver disease causing jaundice and edema.

Physical functionality:

• Alfonse has limited activity and poor exercise tolerance secondary to a long period of decompensation and deconditioning. 

• Alfonse can transfer from bed to chair using a forearm support frame (FASF) and assistance to stand.

• He is fatigued easily and experiences 7/10 pain from his joints when moving or sitting for long periods of time. Pain is 3/10 when resting in bed. 

• Full assistance required for ADLs and administration of medications.

• He requires reading glasses and is slightly deaf.

Mental health:

• Alfonse has episodes of confusion, generally in the afternoon or early evening (his cognitive capacity has improved since the encephalopathy was treated).

• Alfonse has had anxiety and depression since his early teens but is not currently receiving treatment or medication for this.

• Alfonse has a strong and supportive network of family and friends but has not been able to socialize as much as he used to (he states he is feeling disconnected from his old life).

• Alfonse is upset by the changes the ascites have caused in his body – his clothing doesn’t fit and he feels ‘old and fat’. This has made him reluctant to socialize, causing him to withdraw from his social network.

Cultural and spiritual health: 

• Alfonse is from Poland, he immigrated in 1952 with his partner Maksim. 

• English is his second language.

• Alfonse and Maksim are Jains. He believes in an eternal soul that is reincarnated through enlightenment, non-violence, and compassion for all living things. 

• Alfonse has a strong sense of self. 

• He is vegetarian and has recently increased his protein intake in accordance with his doctor’s orders (ascites).

• Food and family gatherings are very important to him.

• Alfonse has resilience and good life skills. He states his quality of life is generally very good.

Social networks:

• Maksim and Alfonse host dinners every week for friends and family. Lately, these have been rotated between other members due to Alfonse’s hospitalization.

• Alfonse belongs to several community volunteer groups, one of which recently honored him with a service medal. He regards his fellow volunteers as a family and they visit him regularly.

• He has a strong relationship with Maksim (his words) and they have not spent more than two days apart since they met. He is worried about how Maksim will cope with his incapacity and impending death.
