Task : 
You are a new case manager at the Indigo Community Services and Health Hub. You have been assigned three new clients as described below. The Team Leader has asked you to prepare for their assessments and provide her with written notes explaining your preparations for interviewing each of the clients.

1. Conduct an assessment for the client/s using a strengths-based approach.  Make a list of each of their needs. Use evidence from the case study to support your identification of these needs.  For each client, identify the strengths and needs for each area and if a referral is recommended.  

2. Describe how you would you prepare for and conduct the assessment.  Consider setting, assessment tools, roles, methods of gathering information etc.  Your answer should explain how your preparation and assessment considers the individual needs of each client. 

3. What are the co-existing needs for this client?  Describe how these co-existing issues impact the client.  Explain how the co-existing needs impact the assessment and planning process.

4. What 3 services or supports are identified through the assessment as most needed?  Explain the basic features of those services and how they match the identified needs and issues.

5. Explain what you need to consider for each client so you can ensure that the client is:
