Assessment Event Overview

The aim of this assessment is to assess your skills and knowledge in the diverse and multi-faceted needs of people and determine both internal and external services required to meet those needs.

This assessment is in 2 parts:

• Prepare and complete client file reviews
• Reflection with another peer/teacher/assessor

Part 1: Prepare and complete client file reviews

Case Assessment

Conduct a thorough case assessment of the requirements of 2 people with co-existing needs, based on their review of the client files provided. You will be provided with templates to record your assessment which includes prompts and questions for you to respond to.

To do this you will need to:

• Gather existing information about each person by reviewing:
  • Ron Jones client file
  • Holly Brown client file
• Seek additional information to ascertain the range of issues the person is experiencing by consulting with a colleague. For this activity the assessor will arrange 2 students to work as colleagues to discuss the file review, to ascertain information you may have not identified. Refer to the class Teams site for further information.
• Using a variety of sources, identify and research the issues this person is facing.

From your review of each person’s client file, your research and your discussions with colleagues and specialists, you are to outline:

• a minimum of 2 co-existing needs the person is experiencing, and an analysis of the interrelationship between these needs
• how these needs impact the person
• issues the person is facing
• strengths the person has shown.

Based on your assessment and analysis of the person’s co-existing needs, using a strength-based approach:

• identify strategies appropriate for the person to consider
• gather appropriate information about services relevant to the person that you will provide to them during their assessment meeting
• identify appropriate referrals you will discuss with your assessor
• detail how the person’s information is to be stored and ensure confidentiality is maintained.

Part 2: Reflection

In consultation with a peer, you will:

1. Outline the steps you have documented in the template provided in Part 1

2. Discuss how you would prepare for the meeting for both clients

3. Review the completed template provided in Part 2 during the meeting

4. Discuss each individual plan and provide feedback

5. Make notes in the template provided below of the outcome of the meeting.
