• Stella is a hostel resident. She experiences problems with her eyes and has had eye drops prescribed by her
  doctor.
• Alice, an experienced ECA, was showering Stella recently and Stella told her that these eye drops were rarely applied into her eyes.
• Alice immediately went to the RN, who confirmed that the prescription has been obtained.  Unfortunately, an oversight in recording the instructions for carers to assist with the eye drops has occurred.
