Provide a timeline of significant events in this young person’s life which you have identified have had significant impact on their social emotional or physical well-being; also include specific incidents that brought them into contact with the helping services.
Discuss how this young person/family came to be working with your service and outline the history of your own working relationships with the young person. Include in your discussion each of the following topics:
What information was given to your service about that young person’s situation and needs when they were first referred to your agency? (You may need to ask your supervisor about how this young person came to be referred to your service and the details of that referral.)
How much of this information was shared with you before you started to work with this young person?
From the information you were given, did you feel confident that you would be able to offer this child /young person and their family the service they needed?
Explain how you used supervision or peer support to assist you in the early stages of getting to know this person this young person
What other services or agencies are involved with this child and his /her family? If you identified the need for extra services to meet the specific needs of this child/young person, how would you go about locating that service and making the referral.
In getting to know their clients, many workers use specific tools for gathering information about young people and their families to help them make some assessments about the strengths and needs of this young person. In this section you will need to complete the following tasks:
Describe any recording and assessment tools (for example, forms, questions, observations sheets, interview processes) that you used or have developed to come to an assessment of the developmental status of the child/young person as well as the strength and needs of your clients and reflect on how you used these tools.
