Ph 0415 269 258), is a Lead Educator at Bright Smiles Childcare Centre (45 Harry Road, Woodmere, 4353. Ph 3225 6598, Email, woodmere@brightsmileschildcare.com.au). She has been feeling unwell the last few days with a runny nose and a fever at night time. She has arrived at the centre for her 9.00am shift, Wednesday 13 June 2017, and has come to you, the Nominated Supervisor, to complain of fatigue and a deep cough and the inability to breathe normally. You check Staying Healthy in Childcare and think that she could have Whooping Cough (Pertussis). You check her employee file and find that she is not vaccinated against the disease. You call Sophie’s husband, Michael Shelton, and organise for him to come and take her to a doctor immediately. You also contact your local QLD Public Health Unit for advice on what to do next to manage this possible serious outbreak. Two hours later you receive a call from Michael, advising that Sophie has been admitted to hospital with confirmed whopping cough. You are required to complete a WHS incident report to lodge to WorkCover.

2. c) Explain what the workplace procedure and legislative requirements are for maintaining WHS incident reports.

Task 3 Risk Assessment for Excursions You are the nominated supervisor at BB’s Child Care Centre, 66 Ernest Street, Southbank QLD. You have has identified in your QIP the need to improve risk assessment documents for service excursions. You need to complete the Excursion Risk Management Plan, including the Risk Matrix, in full for your upcoming excursion, which is to occur in 3 weeks from today, to the Riverside Green Playground at Southbank Parklands. You are taking your Kindergarten class of 21 children, aged four (4) to Five (5) years, to the park for a play and a picnic morning tea as a follow up to the children’s interests in outdoors. You will be accompanied on the day by Jessica Jones, your assistant, Jonah Smith as well as 4 parent volunteers. You are planning on leaving at 8.45am to arrive at the park at 9am. At 11am you leave the park to return to the centre at 11.15am. The notice is then shared with parents prior to the event. You will need to input all information to ensure your risk assessment is complete.
