Task 1

Choose 7 early intervention and/or health prevention and/or health promotion program to address the needs of a specific group/community as advised by your Teacher.

You are then required to create a program targeting support the needs of the specific group. You are required to identify the possible program and conduct the relevant research to begin the planning phase. 

You are required to:

• Research the specific area that your program will target, identify the need in the community, use relevant data such as statistics etc to support the need for the program.

• Compare similar programs that may currently exist and summaries the effectiveness of the program to the identified target group.

• Identify relevant stakeholders who you think would be involved in the program and explain their role.

• Set aims, objectives and strategies for the program research.

Task 2 

Develop a plan to set the program up in the Macquarie Fields Student Community Centre. You be required to address the following: 

• Described the features of your program in line with evidenced based promotion, prevention and early intervention programs.

• Identify the aims and objectives of the program

• Resources (administrative, financial, human, Physical) required undertaking the program.

• Sustainability of the program into the future

• Access to the program

• Accountability and reporting requirements

• Contingency protocols

• Monitoring and Evaluation requirements

Task 3 

Describe how you would move this program from the planning phase to the implementation phase using an action plan identifying activities, resources and timeframes 

Task 4 

Set up a monitoring strategy and include the tools you would use to monitor the program. 

Complete evaluation tools that will measure the overall success of the program. 

Identify and record a strategy for informing key stakeholders of the program of the outcomes of the progressing monitoring and evaluation of the program. 

This information is to be presented in whatever creative format you choose.

For example you could provide a Powerpoint presentation or a written report.
