Mrs. Harriet Field is an 85-year-old woman admitted following a fall at home resulting in a fractured right hip (# R) NOF or neck of femur). She lives alone in her own home, without any community supports.  Her daughter, Susan lives in the country, a two-hour drive away. She phones her mum once a week and visits her every two months. Mrs. Field has been on the rehabilitation ward for two days and the staff is concerned about her weight loss and general well-being. She is malnourished and has had several falls in the past, however, this is the first requiring hospitalisation. She is eager to return home as soon as possible. Planning for discharge Mrs. Field is an 84-year-old widow who lives in her own home alone. Her daughter Susan lives two hours away on a busy family farm. Mrs. Field drives to her local supermarket once a week; otherwise, she spends the majority of the week at home, alone. She manages all activities of daily living independently however she has been relying on; easy to prepare food; rather than cooking for herself. Mrs. Field fell at home whilst hanging out the washing and lay on the ground for six hours until a neighbor returned home from work and heard her calling out for help.Mrs. A field is an immensely private lady and ordinarily doesn’t like having ‘strangers’ coming into her home but given this last scare, she is willing to discuss community support visits.
