Background:

Chronic diseases are one of the leading causes of morbidity and mortality in Australia and worldwide. They pose a significant burden to health services, communities and individuals. Some of the more well-known chronic diseases include heart disease, stroke, diabetes, respiratory diseases and cancers. In 2014-15, more than 50 per cent of Australians reported having at least one chronic condition, and 23 percent having two or more chronic conditions. Chronic disease accounted for 83 percent of all premature deaths in Australia in 2007 (AHMAC, 2017).

A preventative focus on chronic disease can help reduce the burden. Risk factors contribute to the development of chronic disease, and a reduction in modifiable risk factors can significantly improve outcomes. For example, physical activity, dietary intake, alcohol and tobacco use (Halpin et al. 2010, AHMAC, 2017). People are living longer with chronic disease as they begin to appear earlier in life, which results in a greater portion of the population relying on health services for a longer period of time. Prevention and appropriate management of chronic disease is needed to provide a sustainable health system (AHMAC, 2017).

The data collected from the Victorian health population survey in 2017 indicates that this LGA has a statistically significant higher proportion of people living with two or more chronic diseases than the state average.  There are also numerous risk factors, such as obesity, that are evident and contributing to this burden of disease. (Health Vic. 2017). Running a successful campaign to promote awareness of chronic diseases and its risk factors and how to optimise management will contribute to reducing the burden on the health service, as well as improve outcomes for the local community. 

Proposal:

The project will be aimed at increasing awareness of chronic disease in the community and improving outcomes for those suffering from a chronic disease. 

The project will:

• Promote awareness over a week-long campaign. 

• Advertising/promotional material will be produced to be spread across social media platforms, local library/council pages, health service social medias, local tv and radio community announcements and local newspapers to target a diverse population 

• Provide information regarding local health services and those that offer management for chronic disease. 

• Offer pop-up screening/information booths to provide information about ways to reduce risk (eg where safe exercise/walking areas are, community exercise groups, nutrition information etc) and simple screening using evidence based tools around health status, nutrition, physical activity, and body weight/BMI. Pop-ups to be located for example in community venue such as library, local markets or community centres. 

• Liaise with local stakeholders to promote and include in the improvement of the community health – for example, local council and promotion of walking tracks and exercise facility’s that are free and a great way to begin increasing physical activity.
