The assessment is about a chronic illness or disability to discover the impact thy have in their life. Identify two (2) care priorities for your interviewee. The priorities in this case are from the patient  (interviewee) point of view. Not from the point of health care provider. Present each priority using the Levett-Jones (2018) Clinical Reasoning Cycle (CRC), completing the cycle for each priority as outlined in the paragraph structure section of this document.  For each priority, identify goal and rationalise where the person sits on the Roper-Logan-Tierney  (RLT) model of nursing independence to dependence continuum. It is expected that appropriate  evidence-based literature will be used to support the assignment. Need at least 2 interventions for each care priority. 

Interviewee conditions 

• Victoria (pseudonym of the interviewee) is a 68-year-old female lives in Tasmania (cold area) with  David her husband (66-year-old) with Harper which is her daughter. She has 2 grandchild, 2 years  and 4 months old. She has multiple chronic illness. The interviewee conditions are: 
•  Pre-diabetes with poor self-management has been currently diagnosed for 4 months. - hypertension has been diagnosed for 8 years. 
•  osteoarthritis (not taking medication for osteoarthritis) on cervical spondylosis, knee and  arm joint but have done knee replacement 5 year ago. 
• She cannot sit on the floor or squad do due to unable to bend completely. - Increased pain on arm due to carrying newly born grandchild (her daughter stops her - Overweight (Heigh 158cm, Weight 68kg, BMI – 27.2) 
•  has limited movement (Sometime need to use walking stick for mobilization) - Have history of fall (3months ago) near the house while walking and have some injury (skin tare and bruise) 
•  Difficulties in communication due to English is not her language. 

1st priorities – Pain management  

•  Affect sleeping due to pain (around 1 or 2 on pain scale) 
•  Affect mobilization (dependent on walking stick) 
•  High risk of fall (due to cervical spondylosis, easily tripped while walking, unbalance) o Please add if there is any other points 

2nd priorities – Dietary (prediabetes management) 

•  Developing diabetes 
•  Possibilities to develop serious condition of hypertension and other related diseases. - Obesity can affect pain.

Para 4: What is your second priority? What information have you used to arrive at this? Where on  the RLT dependence/independence continuum (by way of equipment, treatments, or interventions)  Be specific and explain the priority in depth here. 

Para 5: What will the consequences be if you do not address this as your priority? This is where you  need to justify this being your priority- use evidence to support.
