1. Rationale

Provide a rationale for choosing the topic for the clinical or programme review

2. Background information on the facility/setting where the review was conducted

Provide relevant background information about the facility where you are conducting the clinical review or the district/sub-district or other setting where you are conducting the programme review. This may include a description of the facility, processes relevant to your review, available routinely collected data relevant to setting the scene etc.

3. Selection of standards or criteria (clinical review) or programme or sub-programme and targets (programmatic review)

Clinical review: Provide a list and/or a summary of the guidelines or criteria that you are using during your review to measure clinical performance against

Programmatic review: Provide a list of programmatic aspects and/or targets that you are using during your review to measure programmatic performance against

 4. Data collection

Provide a description of the process of data collection (methodology) and the definitions of the data elements and indicators (including numerators and denominators) you are using for your clinical review. Refer to your approved topic submission template and ensure you incorporate any lecturer feedback in your full assignment.

5. Data analysis

Provide the results of your clinical or progamme review, including raw data and calculated indicators (e.g. rates or proportions). Summarise the key data using graphs and/or tables as relevant 10 Analyse the data against the standards/quidelines/criteria set.

6. Reflection

Reflect on the findings of your review and what the meaning/implications of your findings is/are for the clinical practice or programme under review. Include reflections on the process of the review and your learning experiences as relevant.

7.  Recommendations and implementation plan

Provide appropriate recommendations on how to address the challenges identified in your review, and provide practical action points as part of an implementation plan.
