Description

This assessment requires students to explore a case in depth to appreciate its complexity and to explore nursing assessment and management in acute and end-of-life phases. 

Questions

1. Applying clinical reasoning and scientific knowing, illustrate how a diagnosis of post-operative sepsis explains Sam’s current clinical status, and integrate specific health data from the case to highlight key points (ULO 1 & 2). This will be presented as a 1-page concept map or flow diagram and a 1-page written explanation (double-spaced, font size with in-text citations) of key concepts/ideas in your concept map (750 words maximum total for concept map and explanation). 

2. Sam is unresponsive, receiving light intravenous sedation, vasopressors and mechanical ventilation with further decline noted on biochemistry and clinical examination. Jenny, as Sam’s Enduring Power of Attorney, has consented to the withdrawal of intensive treatment for Sam. Utilizing McCance's (2016) Person-Centred Practice Framework (below), develop two (2) detailed and distinct examples of how you as the nurse could implement Person-Centred Processes to achieve person-centered outcomes for this family.

3. Sam is now receiving end-of-life care in the HDU area of the Intensive Care Unit. Consider Sam and Jenny's care needs at this time and describe how the interprofessional team might support the best possible outcome with consideration of the current goals of care and the scope of practice of the different professions. The interprofessional team includes medical officers, nurses, dieticians, occupational therapist, palliative care nurse practitioners, pharmacist, social worker, physiotherapist, pastoral support worker and speech pathologist.
