Person-focused reflection
Critically reflect on the experience of a person (and their family/community) you have cared for who is approaching the end of life (the last year of life). Your reflection and analysis of the person’s experience should demonstrate a well-developed application of the concepts from modules 1 and 2 (recognising the last year of life and responding to peoples’ needs at the end of life) to the patient’s personal experience.
Analyse the changing patterns of death and dying in the community of the individual, reviewing how these have influenced this person’s experience and care.
You should describe your role in their care, critically appraising with reference to evidence-based literature, how this person is cared for.
Your description of their experience and journey should incorporate an understanding of their goals of care and evaluate how these (if articulated) contributed to planning their care.
Evaluate the effectiveness of communication strategies that were employed in the care of the individual, their family and carers; the
Renegotiating and Reinforcing Appropriate Care interventions to support the person’s physical, mental, social, emotional and spiritual wellbeing, and the strategies used in mitigating patient, family and carer distress, with reference to evidence-based literature.
Reflect on the outcomes of these interventions and their personal and significant impact on you as a health care provider, and how this will influence your professional practice.
