The aim of this literature review is to evaluate evidence available on the prevalence of discharge delays across acute health care settings, barriers to safe and timely discharge and identify potential initiatives to counteract these barriers. This aligns with the question identified in this project which is, how can ward 5D coordinate safe and timely discharge processes to reduce the length of stay of inpatients occupying the unit. 2. Method Key Terminology For the purposes of this literature review, the following definitions are used: Cummings et al.’s definition of the term ‘discharge’ as ‘the processes, tools and techniques by which an episode of treatment and or care to a patient is formally concluded by a health professional, health provider organisation or individual’ (2010, p. 29).
