Based on this assumption, the first aim of this report is to assess psychosocial factors that affect health status among the residents in Horizon. Secondly, the report will address one of main health issues; how type 2 diabetes is related to the social determinants of health in Horizon and why it needs to be considered as a health priority in the community. The last purpose of this report is to show various roles of the health professional, especially the pharmacist, with three key strategies of advocacy, enabling and mediating, in order to tackle the diabetes issues and deliver the best healthcare services to community members.

The rate of residents aged over 65 in Horizon is 34.8 percent, which is much higher than that of other regional areas in South Australia. (City of Horizon 2018, p.10).
