Overview

Aged and Disability care services are often directed by funding bodies to ‘partner’ with service users (clients). Service directives use words suchas 'choice' or 'supported decision making' to emphasise this partnership. Typically, this is articulated in services such as Consumer Directed Care (CDC), Home Care Packages, and the National Disability Insurance Scheme,and others. Underpinning this expectation is an assumption that a service provider will practice in an inclusive way, and that service users will be consulted and offered choices about the way their service is provided.

In keeping with these social expectations, Dementia Australia states that we need to rethink the way we work with people with behavioural and cognitive symptoms. We should have an expectation that people can participate, to their ability, in decision-making. You will find similar advice from other organisations that provide support to younger people with disabilities and older people.

Assgnnment Question

Discuss how and why,as service providers, we can maximise opportunities for choice and participation for our clients with behavioural and cognitive symptoms,including the challenges this can pose. How can wedoit, and importantly, why should wedoit?

You will need to select one area of professional practice to orientate your answer. Choose from older people with dementia OR youngerpeople with intellectual disabilities OR younger people with cognitive disabilities.

Case study - Behaviours of Concern

Case Study

You are a employed as a human services consultant and have been asked to review a person’s “challenging behaviour.” This person can be located in either a community, residential, clinical or group home setting. The person has been referred to you for assessment becausetheyare “wandering” and “trying to escape.” Whenthey are stopped bya carer, they becomephysically aggressive, pushing, punching and verbally abusing the carer and those nearby. There are other times however whenthe personis observed to be quiet and withdrawn. The carer/s have raised concerns about their capacity/ability to manage this “behaviour” for much longer and are considering sedation.

Your task is to undertake a holistic biopsychosocial assessmentto recognise the contextual nature of the behaviour being expressed. In your report you will need to utilise the following headings and outline the details.

1. Client Information

  • Details of the person who presents with distressed behaviours. Include in your assignmenttheir fictional name, age and gender
  • The setting - community, residential, clinical or group homesetting
  • Outline any considerations relating to this person that needs to be consideredin the assessmentprocess. This can include; being a refugee, identifying as LGBTIQ,a victim survivor of family violence, being culturally diverse, religious considerations etc
  • Outline any consideration as they relate to the context; rural locale, staffing, staffing shortages,financial issues, insecure accommodationetc
  • Any other relevant information required to provide context - for example, health issues, family background, education etc

2. Prepare an ABC Chart

  • Provide an ABC Chart as outlined in Topic 4 (table format) with at least three entries. The purposeofthis is to identify and chart the behaviour.
  • Atleast three line items are to be included. Please note that this chart will be further developed in the assessment section - so detail needs to be consistent with the examplein the I2 notes. + Make of note of the impact of this behaviour on both the person and others

3. Outline the role of assessmentin distress behaviour

4. Outline a holistic and person centred assessment

  • including the biological, psychological, social and environmental contexts influencing the person’s behaviour.It is also importantto link this to the ABC chart and other functional assessments.

5. Some suggested strategies to respond to the distressed behaviours
