Counseling Role-play: Bipolar Disorder
The following role-play is between me as a counselor and my friend Shaun who is 20 years old. Two years back he encountered a lunatic episode followed by a session of depression. He was fine later and gave his final year exams where he experienced an increasingly elevated mood. This made him hospitalized and was followed by an interval of depression.
After a month after his discharge, Shaun comes back for a sitting of psychological treatment:
Me (Counselor): Nice to meet you again Shaun (smiling)! How are you today?
Client: I’m okay
Me: So I understand you had your exams, after that you went to the hospital again?
Client: YaMe: So, can you tell me what happened before that
Client: Umm.. about a few months ago now! I’ve been not doing well past four years
Me: Ya, I've not seen you for long
Client: Four years back…. I was a bit disappointed to go through it all again ..Aaa but it’s so I started preparing for my university exams and took up more work. Stress level increased (gulps), I had a cough so for that took cough tablets (gulps), had energy drinks. Because the environment was like that a lot of people tried to push them, I took Vitalin (a nutritional supplement and for better brain functioning)
Me: Oh wow! Did you take Vitalin!?
Client: Ya, because I use to…Aaaa that was necessary at the time, I required …(puzzled expression)
Me: How did that affect you?
Client: Well I, ….I saw that I got more confidence, everything was going on, I became a lot more fruitful, I was working hard and my mind was working swiftly. Also, m….my pace was gushing out until the period, so yes.. yes I was doing my work.
Me: So, what does that mean to be! As it sounds like you put a lot of pressure on yourself, at that time what you felt like more kind of awaken or energized maybe.
Client: yeah well, for me it was a tough phase. So it’s been three years, I never talked to my buddies about it. But then I ya started to … and I… grabbed the chance to ya relish this time. Umm.. and it did not hit the bottom line till my examination. I was still performing umm for my exams and I have since discovered that I didn’t achieve outcome I wanted to.
Me: Hmm! What do you think that was?
Client: (Scratching his arm) Well, I believe I was diligent but Ahh... There was a mysterious thing that I didn't perform as per my capability so that I can achieve the first rank.
Me: What else was happening around you when you took the stress of your exams (wide eyes) and staying awake for them. Was anything else bothering you at the same phase?
Client: Well, that was the end of my university and I… and that was the duration where free hours start I stopped working. I wanted to work further…So that’s why to boost my energy I took vitalin (scratches head) and started a sort of substance abuse, started taking alcohol… took a bit.
Me: okay
Client: Then I just saw I grew into a person I thought was him was… a guy wants to be like (clicks tongue) insane and everywhere
Me: So this is the time after your exams and before your results. Right? A few weeks
Shaun: Yeah…ya yaMe: So I started using it all night..ummClient: You said you were taking Vitalin intrigue of coffee and using other drugs as well.
Me: Yeah, so I developed (scratches head) into Cocaine, lot cocaine (Low voice), MDMA (3,4Methylenedioxy?methamphetamine) aaa…
Me: And you do it affect you?
Client: Frankly I was on the top of the world (Smiling) and yeah I was just always an individual everybody wanted to understand. I imagined myself in the center of the universe and I even persuaded the friend of mine who develops apps (laughed), Ummm.. that we should, have not yet! But should that when we are going into the alcohol shop that it was the most cost-effective way to get through it
Me: Right
Client: ya, I mean the time I was spending heavily, my parents saving... I got a credit card which I never tell them about. So in this part the man I became I spent a lot on alcohol, narcotics, attires, and this gizmos, and ya I was.
Me: (soft tone) So it sounds like it started with the exam pressure and just to keep yourself awake. And then you began to feel like you are a popular man, more successful, how did you respond to those symptoms that you experienced then?
Client: Well having no hard to deal with my illness as I was not a part of that world, I didn't tackle those signs, in a way, it's gone a have long-term steadiness. It began with doing the work I needed to achieve and I just let it take hold, it wasn't someone else. It was me. I was the master of that situation.
Me: So, it sounds like the one who didn't confront and let it roll and on the contrary you said that you are very energetic and powerful. I mean how that affects you, looking the things in that way
Client: (Thought a while) I just.. it was my time, like become a person I wanted to be, all the hard work I have done and my parents did for me it felt like all is crystallizing.
Me: (eyes wide and smiling) It's where you are at the brink of something that sounds well. Talking about the duration when you started taking refreshers, I mean how much sleep you were getting at the starting.
Client: It slowly reduced, initially it was five and ended up in three. I think the laborious I was working the rapidly my brain then would work, muscles were firing and my goals were stronger and I got more determination.
Me: and how that it affected your body physically
Shaun: No, I can recall that I was avoiding confronting but nothing powerful comes at the line.
Me: Okay, so recall the things that happened after your exam, apart from what you did, was there something else you did to be more powerful?
Client: I focused on invested my parents saving that isn't with me anymore, all used on drugs. I was only my main character; I gave myself importance and felt good (shaking voice)
Me: Okay.
