(You must complete a permission to observe form signed by the supervisor and parents, ensure the child’s real name and  Service name are not used- all personal information must remain confidential). 

2. Record four (4) observations at your workplace (using template 1 x 4 ) OR
3.  Alternatively, students can complete this assessment task by watching two (2) vignettes on a children
4. Complete your observations and record in detail the actions and words of the children involved in the interaction/s. Consider the frequency, intensity, and duration of children’s behaviors you are recording to ensure you have included sufficient detail. 

5. Identify the child’s behavior/s of concern that require support. (use template 2)Using point form – Identify strengths of the child, list any behavior/s that you believe are concerning, based on the observations you have recorded. 

6. Identify influences of the behaviors requiring support (Use template 2)

• Clearly identify the factors or triggers that may be causing the behavior/s.
• Identify any characteristics of the environment that may be contributing to the behavior.
• Identify how learning difficulties or developmental challenges or mental health issues may have affected this child’s behavior.
