• A formulation of the problems that include the symptoms of mental illness, the context of the admission to acute care, vulnerabilities and strengths While this is a clinically-based assignment, your assignment must: 

• Be supported with relevant references (avoid a reliance on health policy documents as your sole source)

• Demonstrate your capacity for sound clinical reasoning

Use a Bio psycho social assessment framework to collect your cues about this person. It will help you to gather in a logical, sequential manner information  about both person and their context. For example do they live alone? have a support system? do they smoke (nicotine, illicit subtsances)  drink alcohol? any stressors eg. significant loss, are they employed, chronic pain? what is their mental health literacy like- take their medications? lifestyle factors (exercise, diet, sleep), do they want to get well? have hopes/ dreams. How do they self care when they are stressed/ coping mechanisms. Is there a relapse prevention plan? I am just listing off the top of my head to prompt you but use an assessment tool to guide you.
